


















AgeOptions – Food Support for Older Adults
AgeOptions offers evidence-based falls prevention and health promotion programs to West Cook Coalition members. These group programs take place throughout the West Cook Coalition area. We offer four different falls prevention programming to address the different needs of older adults. These include, A Matter of Balance, Fit & Strong!, Bingocize and Tai Chi for Arthritis and Falls Prevention. Our health promotion programs are called the Take Charge of Your Health Programs and offer information and tools so that people can make meaningful changes to their health.

BEDS Plus Care – Housing Support
BEDS Plus Care provides housing and supportive services for West Cook Coalition members experiencing homelessness who have severe, chronic health conditions. The program offers safe, dignified housing for medically vulnerable adults while they stabilize, recover, and work toward permanent housing. By providing interim, bridge, and transitional housing options with on-site support and care coordination, BEDS Plus Care helps reduce avoidable emergency room use and hospital readmissions and supports long-term health and housing stability.

Beyond Hunger – Food Support
Beyond Hunger provides food assistance to West Cook Coalition members experiencing food insecurity through its food pantry and mobile food pantry. Services include pop-up mobile pantry sites in underserved communities and access to pantry food at no cost to eligible members. Through WCC support, members may receive food assistance to help meet household needs for an average of up to four months.

Care Advisors – Social Care Management & Data Coordination
Care Advisors is a minority-owned social care management organization that supports West Cook Coalition’s efforts to reduce health disparities through coordinated social care. Care Advisors leads WCC’s social care management initiatives and provides a secure, web-based platform that enables data sharing across clinical providers and community-based organizations. This interoperability supports coordinated care, improves communication among partners, and helps ensure members receive timely, appropriate services across health and human service systems.

Catholic Charities
Catholic Charities is developing a new multi-service community center in Summit, Illinois, designed to expand access to essential food and dining services for West Cook Coalition members. Through this capital project, the new site will support an enhanced food pantry and congregate dining area, creating a welcoming space that strengthens food access and community support. Community Support Services (CSS) – Developmental Disability Specialty Care
Community Support Services provides support for West Cook Coalition members with intellectual and developmental disabilities and their families. Family Focus – Maternal and Child Health Services

Loyola Physician Partners(LPP) brings deep expertise in value-based care and advanced payment models, with a strong history of participation in CMS Medicare ACOs, Medicare Advantage, and bundled care initiatives.
