The West Cook Coalition (WCC) is a collaborative network of healthcare providers, social service organizations, and community partners working together to transform health outcomes in west Cook County. Led by CareAdvisors, our coalition focuses on six core strategies: community navigation, disease management, care coordination, social care integration, behavioral health access, and community-based services for vulnerable populations. Our goal is to create a seamless ecosystem that addresses both clinical and social needs.
