West Cook Coalition was created in 2023 with funding and guidance from the Healthcare Transformation Collaboratives (HTC) program. Created by state law and administered by the Illinois Department of Healthcare and Family Services, the program establishes locally integrated networks of health and social service providers to launch transportation programs that reduce care gaps and healthcare disparities in distressed communities throughout Illinois. In particular, the HTC program seeks to increase access to community-based services and preventive care and address health-related social needs. HTCs operate throughout Illinois.
